Psychologists call this phenomenon "habituation," and it's not limited to homes. It can also happen in busy family medicine practices. Usually the unsightly details in medical practices are small, and thus easy to overlook by busy physicians and staff members focused on patient care.

However, patients are looking. Small details and first impressions have the potential to create a powerful set of assumptions. If patients see something askew or neglected, it may plant seeds of doubt in their minds about the practice's quality of care.

The observations in this article are based on data from the TransforMED national demonstration project (for more information, see "TransforMED Tries to Rebuild Family Medicine," FPM, May 2007).
