Article Excerpt Of almost 200,000 medication errors in 482 U.S. hospitals, more than 3,000 (1.67 percent) caused harm, including death. The U.S. Pharmacopeia (USP) issued a report with these findings, based on 2002 data, in November. The report also found that senior citizens were twice as likely to be harmed by medication errors, with seniors accounting for 11 of 20 reported fatalities.
The USP is an independent nonprofit organization that sets standards for medicines and runs MEDMARX, an anonymous, voluntary hospital-medication-error reporting system.
