The case presented here serves as a reminder of the importance of differentiating hemoptysis from pseudohemoptysis. In most cases, this can be accomplished by a careful history and a thorough physical examination with special emphasis on the upper airway and GI and respiratory tracts.

The case

A 24-year-old man with no past medical history was admitted to the hospital for evaluation of daily, mild (less than 50 mL) hemoptysis of 1 month's duration. He also had nasal congestion, hoarseness, and weight loss of 20 lb.

Physical examination revealed left submandibular lymphadenopathy. Findings on the patient's chest radiograph were normal. A large nasopharyngeal mass was found on direct laryngoscopy. Results of a biopsy were consistent with poorly undifferentiated nasopharyngeal carcinoma. The patient's MRI scan is shown in the Figure.
