Human error caused the April 2004 fatal explosion at Formosa Plastics' Illiopolis, IL plant, but "inadequate planning" contributed to the severity of the accident, according to a final accident investigation report by the Chemical Safety and Hazard Investigation Board (CSB; Washington). The explosion killed five workers, injured three, and caused the evacuation of about 150 area residents. "People do make mistakes, and that is why it is all the more important for chemical plants to design systems that take into account the possibility of such errors," says CSB chairman Carolyn W. Merritt.
