The latest Institute of Medicine report on medication errors reveals that not much has improved since the 1999 IOM report To Err is Human drew national attention to the widespread problem. This form of medical mistake still harms 1.5 million patients per year, costing upward of $3.5 billion.

Not long after To Err is Human was published, our facility, 460-bed McLeod Regional Medical Center, initiated a series of steps to improve patient safety and to reduce the hospital's "rate of harm.'' The hospital didn't want to wait for another IOM report or new state or federal guidelines to address the issue. Instead, a significant top-to-bottom culture change was launched, including the use of new technologies and an overhaul of the medication delivery system.

As a result, we have achieved our goal of reducing harm to less than one per 1,000 doses, and we've cut the number of steps in filling...
