At least one thing is consistent about power in healthcare: Its true source does not vary much from year to year.

Sure, some of the names change-politicians and political appointees come and go, their influence entirely dependent on the office or position they occupy. But even with these inevitable shifts, there's a certain longevity in what might be called the ruling class of American healthcare, at least according to the results of Modern Healthcare's annual list of the industry's 100 most powerful men and women.

This year's compendium of healthcare clout, which is led by multibillionaire Bill Gates, includes 23 individuals whose names have appeared on all five rankings of the 100 Most Powerful People in Healthcare since the first roster was published in 2002.
