While Japan trails behind the United States in the widespread practice of family medicine, there are moves among some Japanese physicians in private practice to undergo clinical training in areas outside their specialties and in hospitals to set up internships for physicians who want to treat individual patients in their offices on a regular basis.

In 1985, the then Health and Welfare Ministry studied the possibility of introducing such a system but failed to realize it in the face of opposition from groups such as the powerful Japan Medical Association (JMA).
