Even as Anthem Inc. made a highly successful debut as one of the nation's largest publicly-held health insurance companies, it has encountered criticism.

A growing number of physicians and health providers are challenging Anthem, complaining that the insurance giant's reimbursement policies are shortchanging them.

Those complaints, have resulted in an Indiana Department of Insurance examination of the insurer's business practices and a spate of stories about other physicians and providers unhappy with Anthem.
